"Order By" is your billing information.
"Deliver To" is the name of the person you want the Gift Certificate
made out to.
"Deliver To Address" is where you want the gift certificate mailed
to.
Note: If you want the Gift Certificate held here at the store you must
write "Hold At Store" in the "Additional Information" section of the
checkout form. Otherwise it will be mailed to your "Deliver
To" address. Also, the "Order By" name will be on the
certificate to show who the certificate came from. If you
DO NOT want your name on the certificate, you must write
"No Name" in the "Additional Information" section of the
checkout form.
